Participant/Parent Pop Warner Agreement
​All participants participating in the 2017 season must have this form completely filled out and signed  prior to the first practice.

Participant and Parent must read and complete prior to stepping on the field for practice.

Physical  / Medical Release
A participant must provide a completed  2017 Pop Warner Physical / Medical Release form prior to stepping on the field for practice.  It must be dated after 1/1/17.

Copy of 4th Quarter 2016 Report Card
4 Copies of the participant’s completed 4th quarter 2016/2017 year report card must be submitted.

Copy of Birth Certificate
​All participants must submit a copy of their birth certificate. (Note: Original Birth Certificates must be provided upon advancement to post season play.
i.e. championship games)




Our 2017 Fall Football "Early Bird" registration begins on the following dates:

  • May 15th - Great Bridge Community Center (6p - 8p)
  • May 20th - Great Bridge Community Center (2p - 4p)

  • May 25th- Greenbrier Library (6p - 8p)



Please read all details below as there are some options that may have changed.

Completion of Registration Includes:

  • Registration Fee:
    • $180 Regular Registration 
    • Full payment due at registration. Non-refundable upon receipt. No exceptions. No checks.
    • Credit cards accepted with a  processing fee.
  •  Season Mandatory "Fun-Raisers" or Buy Out Option
  • Paperwork Compliance completed prior to August 1st practice at any of our onsite registration/paperwork turn in dates. (See paperwork list below)
  • Equipment Fitting and Handout​ (Onsite during practice)

Note: For the 2017 season the Chesapeake Knights will hold ONE Mandatory "Fun-Raisers."  Families will be required to participate or pay $75 per child to Buy-Out  for the "Fun-Raiser."  Cash or credit only for buyout. No Checks accepted. Processing fees apply to credit/debit cards.

The Registration Fee Includes:​ 

  • Uniform Rental of Helmet, Chin Strap, Game Jersey, Integrated Pad Game Pants, and Shoulder Pads
  • Cheer Uniform Rental (shell, skirt, and pom poms)
    • ​​Accessory Package is separate and includes: bloomers, bow, socks and shoes
  • Facilities and Football Equipment
  • Banquet Admission for Participant and Family
  • Banquet Trophy
  • Field Maintenance
  • Porta-Potties Rental
  • Storage
  • Official Fees​ and Pop Warner Insurance and Assessment Fee
  • Administrative and Registration Supplies Note: Fees do not include costs associated with travel for preseason or post season play, which includes but is not limited to regional and national games, bowl games and elective games before and/or after the beginning/conclusion of the regular season. 

Refund Policy

**All registration fees, fundraising fees, and buy out options are non-refundable upon receipt. No exceptions.
